Help the Hag Survivors is a quest in Act Three of Baldur's Gate 3. It can be started by interacting with an Anti-Hag Support Group Poster in the Lower City.
Objectives
- We found a poster that mentions a meeting for people who have survived hags. It's being held at Barren's Coop.
- We found an eviction notice. The people who lived in Barren's Coop drew the wrath of a hag, and were kicked out by their landlord.
- With Auntie Ethel gone, the people she terrorised are finally safe. We should find them and tell them the good news.
- We found a note in the Blushing Mermaid addressed to 'Auntie' - a term for a hag. The note mentioned Old Garlow's Place, and people who are hiding there from a hag.
- After meeting a disguised hag in the Blushing Mermaid, we found a note nearby addressed to an 'Auntie'. The note also mentions Old Garlow's Place: seemingly a refuge for the hag's victims.
- We found a letter from someone called Kled. In it, he said he found somewhere new for everyone to live - Old Garlow's Place. He wrote that they can hide from the hag there.
- We met a group of people hiding from a hag. Their leader is none other than Mayrina - the pregnant woman we saved from Auntie Ethel. She's been transformed into a sheep by a hex. We need to find a way to help her.
- We found a note penned by Marinya. In it, she mentioned seeing a doll everywhere - right before she turned into a sheep. It must be the key to her curse.
- We spoke to Mayrina in her sheep form. She kept trying to say the word 'doll'. We need to find this doll - it must be connected to her curse.
- We met a group of people hiding from a hag. A doll is tormenting them. We need to find it and destroy it.
- We destroyed the doll.
- Mayrina is human again thanks to us. She's shaken, and revealed that the one responsible for her curse is her fellow survivor - the dragonborn.
- We pickpocketed Jatlo and found a note. It mentions 'Auntie', a 'hex doll', and a 'book'. We should ask him about it.
- The hag survivors in Old Garlow's Place, convinced that we're allies of a hag, attacked us.
- It turns out Jatlo was secretly a redcap who was working for the hag. We need to take him down.
- The dragonborn transformed - revealing himself to be a redcap and an agent of the hag. The hag survivors, realising their mistake, switched sides to help us.
- Once the hag survivors died, the dragonborn suddenly transformed - revealing himself to be a redcap and an agent of the hag.
- We defeated Jatlo. We should speak to the victims.
- We defeated the redcap. There is no trace of the doll now that its owner has vanished. We should speak to the hag survivors.
- We defeated the redcap, and Mayrina immediately transformed back into a human. He must have been the source of the curse. We should speak to her.
- Auntie Ethel is finally dead. The hag survivors will have feelings about this - we should speak to them.
- The hag survivors are grateful to us. With Auntie Ethel dead, they're finally free.
- The hag survivors were shaken by the revelation that one of them was a redcap. They think he was after the book they have hidden in a safe. They offered it to us, saying it will help us kill a hag - permanently.
- Mayrina thanked us for helping to root out the spy in their midst.
- The hag survivors are dead and gone. Old Garlow's Place lies empty.
- The dragonborn transformed - revealing himself to be an agent of the hag. We defeated him, but the other hag survivors also perished.
- Mayrina is dead.
- We left the Lower City, crossing a point of no return, and can no longer help the hag's victims.
Walkthrough
Find the Hag Survivor Group
Upon entering the Lower City, many posters are put up around the city on various noticeboards and walls. One such noticeboard is near Luan, the paperboy at X: 125 Y: -70. Interacting with the Anti-Hag Support Group posters begins the quest and lead to a possible meetup place at Barren's Coop near X: 45 Y: -114. The door to Barren's Coop can be lockpicked open, and passing a Perception check reveals an eviction notice. To find the new location of the hag survivors, the table must be searched, revealing a barely legible letter.
The letter left at Barren's Coop reveals that the group has relocated to Old Garlow's Place at X: -49 Y: -127. As with the past location, the door needs to be lockpicked open. After entering the building, the party encounters the hag survivors, who are instantly suspicious and assume they are dealing with hagspawn.
Persuading the Group
Choosing the following options and successfully passing the checks has the group stand down:
- [PERSUASION] A hag is after you? I can help. I've fought one before. ( DC 15 Persuasion check)
- [INTIMIDATION] Touch me and I'll hurt you more than any bloody hag could. ( Intimidation check)
The group then explains that they are being pursued by a hag. Their leader, Mayrina has been hexed, and they need assistance. Mayrina is upstairs, and has been transformed into a sheep. She can be talked to using and shares that a doll is what's causing her transformation, or her diary can be read, which also references a doll.
Finding the Doll
The doll in question is the Voodoo Doll at X: -41 Y: -141, which is in the corner of the upstairs room. The doll can be attacked, injuring Mayrina in the process, or safely destroyed using . Afterwards, Mayrina transforms into a human and begins shouting that the Dragonborn did it. The ruse having ended, Jatlo reveals himself as a servant of the hag. He attacks the party and summon minions to help him.
Pickpocketing Jatlo
If Jatlo is pickpocketed, he has a note on him using terms like "Auntie" and "hex doll." After confronting Jatlo, he transforms into a redcap and attacks the party. Defeating Jatlo ends the curse, turning Mayrina back into a human.
Fighting the Group
If the Persuasion or Deception checks fail, or if the party simply decides to fight, then the hag survivor group attacks, certain that they are facing off against hagspawn. However, after Jatlo is attacked, he transforms into a redcap and the other survivors realize they were mistaken. The survivors aid the party in taking down Jatlo and, with his defeat, Mayrina transforms into a human.
Tools to Fight the Hag
Mayrina tells the party that a hag is in the city and it's out to get her. She asks for help in defeating the foul creature, and states that information on how to defeat a hag is in the wall safe on the first floor at X: -61 Y: -148. The safe contains A Hunter's Guide to Hags, Ashes of Dried Fey Flower, and a Tearstained Diary which includes a recipe for Hag's Bane and how to use it. Agreeing to help Mayrina also sets the party on the quest Avenge the Hag Survivors.

Notes
- If the party has already killed the hag, for instance while completing Save Vanra, heading to the Hag Survivors plays out differently. Mayrina won't be hexed, and Jatlo will have turned back into a redcap and killed himself in front of the other survivors. No Ability Checks are necessary and the quest completes after the survivors are informed of the hag's death. Avenge the Hag Survivors is skipped and the party immediately receives the Fey Semblance Amulet, but they do not receive the Staff of Interruption.
- Occasionally this quest may not update when completed and a map marker may remain.
