It's likely the Dream Visitor was originally a manifestation of the tadpole within the player character's mind attempting to break down their will, and successfully transforming or enslaving them to further the Grand Design under the Absolute

This is just a rumor/speculation that's often repeated in internet forums. But it is directly refuted by the game devs themselves; according to director Swen Vincke, "The Emperor was 'always the twist.'" I can't link the interview because it's flagged as spam, but it's from an IGN article titled "Baldur's Gate 3 Developers Explain Its Controversial Endings, Beloved Characters, and Making the Best RPG of 2023"
