If a party member wears it, they'll be possessed by Ethel and attack the party unless they're under the effect of Protection From Evil and Good. If you successfully wear the mask without possession, though, the full party can pass through the door. The masks also prevent the hag servants from becoming hostile, but only towards the masked character(s); any others need to sneak past if you want to avoid confrontation.  If using masks with Protection From Evil and Good, take care to avoid breaking concentration due to taking damage or casting another concentration spell.
