If the page title corresponds to the condition name, then you don't have to provide the name parameter. However, if you have to name a page something like Blah (Condition) to disambiguate it from some other page called Blah (e.g. Chill Touch vs. Chill Touch (Condition)) then you'll want to provide an explicit name = Blah value.

Likewise, if the icon for the condition is called Blah Condition Icon.png then you don't have to provide the icon parameter.

Examples:

Blade Ward (Condition)

Chill Touch (Condition)
