Zgorrellik will show up next to the 
Baldur's Gate waypoint in the Lower City once the party has collected all three Netherstones. Once the party comes near, she will be forcefully transformed into a Mind Flayer using 
Ceremorphosis together with 
Morrus Pinfeather and 
Dubos Clodds.
