The "Mask of Regret" is an NPC in the Overgrown Tunnel. She is magically enslaved by "Auntie Ethel" the Hag.
If Auntie Ethel is killed without killing the Mask of Regret, she says she will return to Baldur's Gate and take action against other Hags to prevent the suffering of other victims.
In Act 3 she can be encountered again as part of the quest Help the Hag Survivors, now named as Hydrangea Wubb.
