Nowhere is she ever referred to as "Shadowheart Hallowleaf." While she was in the cloister, she was never referred to by the surname, as that was against the whole memory-wiping bit. Further, if you want to be like, "Oh, well she gets the name back after rescuing her parents," that is something determinant on the player's choices in the game.
