Rewarded by Lucretious at the end of the Find Dribbles the Clown questline. Alternatively, can be pickpocketed or looted from Lucretious.
Bugs
As of Patch #3 (4.1.1.3732833) the damage gets added to non-attack rolls spells like Magic Missile (for each projectile) or Fireball (every creature affected)
