Myrnath is aboard the Nautiloid and can be found on an upper ledge next to a collection of brain jars, mind sanctuaries, and tentacle aquariums. Upon entering the room he is in, Myrnath seemingly calls out to the player character, asking them to "help us". If approaching, it is quickly revealed that a psionic presence - rather than Myrnath - is communicating with the player character via telepathy. Investigating further reveals that Myrnath's skull is partially removed. His exposed quivering brain is a newborn intellect devourer.
