Deeziks Tormo can be found towards the end of
Act Three in the
Lower City.
Deeziks Tormo shows up just south of the Basilisk Gate in the Lower City once all three Netherstones have been collected by the party. Once the party approach, she is forcefully transformed into a Mind Flayer by 
Ceremorphosis together with 
Muttonchops Dexon and 
Saer Eleganza.
