Disguise Self is a level 1 illusion spell. It allows spellcasters to temporarily change their appearance. While active, gain access to the action Dispel Disguise to end the effect.

- Wizards can scribe a Scroll of Disguise Self to their spellbooks.
Notes
- Disguised companions and hirelings revert to their normal form upon leaving the active party.
- Apart from size, this spell does not grant other aspects or racial features of the assumed form. For example, a Halfling disguised as a Wood Elf still only has 7.5 m movement speed.
- Since the spell changes Creature Size, it can be used to fit through some entrances with size requirements.
- While disguised in the form of a different race, the idle animations of that race are used.
- A separate Attitude score is used (and permanently maintained) with merchants while disguised.
- The version granted by Mask of the Shapeshifter and Eldritch Invocation Mask of Many Faces is called Shapeshift and does not cost any spell slots.
- If used prior to equipping or interacting, Disguise Self allows characters to bypass race restrictions on items (e.g. Silver Sword of the Astral Plane and Dwarven Thrower) and use race-specific dialogue in NPC interactions.
- It allows using Speak with Dead on creatures the caster has personally killed.
- As of Patch 5, it is possible to escape each prison once per character making use of this spell. As long as the appearance changes after being jailed (committing a crime while disguised, then dispelling the effect while in jail, or vice versa), having the Fugitive condition does not cause guards to spot the offending character. Dispelling the effect before the Fugitive Condition ends allows guards to recognize the character. If trying to escape the same prison more than once using this method, guards spot the character no matter what they look like.
Bugs
- Disguise Self disables effects which require the Brand of the Absolute to trigger, such as Gloves of Power.
- As of Patch 5, Hotfix 17, it is possible to give the caster racial equipment bonuses for longer than intended by using the Mask of the Shapeshifter, taking the mask off, and long resting without removing the disguise. On waking up, the disguise is gone, but the racial bonuses on the equipment still affect the character. This only works for racial equipment bonuses, not racial dialogue options.
- If the player character is disguised as a githyanki when talking to Shadowheart for the first time on the beach at the start of Act One they are awarded an extra 30 xp.
- If a companion is disguised while at camp, they will remain where they were last placed when another companion or player character is selected, rather than automatically moving back to their tent, until they are moved again or a long rest is taken. This can be useful when seeking to apply conditions or spells with a limited radius, such as Aid.
- While disguised, hirelings cannot be spoken with.
