This is a comprehensive list of all sources of effects that directly affect and interact with melee Unarmed Strikes.
General information[edit | edit source]
- A source can cause different effects as part of its mechanic, but this article lists only the unarmed strike related portion.
- Items listed in this article do not include mundane gear pieces of common quality and basic enchanted versions thereof.
- If a condition is incurred by a singular source (for ex. a class feature), this article lists that singular source. Otherwise only the condition is documented.
- For conditions caused by gear-exclusive passives and actions, the gear item takes precedence over the passive or action in the list.
General effects[edit | edit source]

Any modifiers to melee attack rolls and damage rolls, or all attack and damage rolls if specified for the source, will also affect unarmed strikes. The exceptions to this are:

Barbarian[edit | edit source]
- Rage - Deal additional 2 (at Barbarian level 1) or 3 (at Barbarian level 9) damage with unarmed strikes.
Druid[edit | edit source]
- Symbiotic Entity (Circle of Spores) - Deal additional 1d6Necrotic damage with unarmed strikes while temporary hit points are active
Monk[edit | edit source]
- Manifestation of Body - Deal additional 1d4 + Wisdom modifierNecrotic damage with unarmed strikes
- Manifestation of Mind - Deal additional 1d4 + Wisdom modifierPsychic damage with unarmed strikes
- Manifestation of Soul - Deal additional 1d4 + Wisdom modifierRadiant damage with unarmed strikes
- Martial Arts: Bonus Unarmed Strike - After attacking with an unarmed strike, monk weapon, or using a monk ability, you may make an additional unarmed strike with a Bonus Action
- Martial Arts: Dextrous Attacks - Allows unarmed strikes to scale with Dexterity or Strength, whichever is higher
- Martial Arts: Deft Strikes - Increases the base unarmed damage from 1 to 1d4 (at Monk level 1), 1d6 (at Monk level 3), or 1d8 (at Monk level 9)
- Fangs of the Fire Snake - Make an unarmed strike that does an additional 1d10Fire damage with an extended 6 m / 20 ft range. Subsequent unarmed attacks during that turn deal additional 1d4Fire damage
- Flurry of Blows - Make two rapid unarmed strikes with a Bonus Action
- Flurry of Blows: Push - Make two rapid unarmed strikes with a Bonus Action and possibly push the target 5 m / 17 ft away
- Flurry of Blows: Stagger - Make two rapid unarmed strikes with a Bonus Action and Stagger the target for 1 turn
- Flurry of Blows: Topple - Make two rapid unarmed strikes with a Bonus Action and possibly knock the target Prone for 1 turn
- Ki-Empowered Strikes - Unarmed strikes count as magical damage for the purpose of overcoming resistances
- Shadow Strike: Unarmed - While Invisible or Sneaking, teleport to a target up to 18 m / 60 ft away, landing an unarmed strike that deals additional 3d8Psychic damage
- Stunning Strike (Unarmed) - Make an unarmed strike and possibly Stun the target for 1 turn
- Tavern Brawler - Apply the Strength modifier to unarmed attack rolls and damage an additional time

- Infernal Fury - After consuming a Soul Coin, Karlach deals additional 2d4Fire damage with unarmed strikes[note 1]
- Vampire Ascendant - After ascending, Astarion deals additional 1d10Necrotic damage with unarmed strikes

Bonus damage[edit | edit source]
- Boots of Uninhibited Kushigo - Deal additional damage equal to your Wisdom modifier with unarmed strikes
- Callous Glow Ring - Deal additional 2Radiant damage against illuminated targets
- Corellon's Grace - Deal additional 1 damage with unarmed strikes, and gain a +1 bonus to unarmed attack rolls
- Gauntlet of the Tyrant - Deal additional 1d4Force damage with unarmed strikes
- Gloves of Cinder and Sizzle - Deal additional 1d4Fire damage with unarmed strikes
- Gloves of Crushing - Deal additional 2Bludgeoning damage with unarmed strikes, and gain a +1 bonus to unarmed attack rolls
- Horns of the Berserker - While not at full health, deal additional 2Necrotic damage[note 2] with your unarmed strikes
- Scabby Pugilist Circlet - When surrounded by 2+ foes, deal additional 2 damage with unarmed strikes
- Seraphic Pugilist Gloves - Deal additional 1d4Radiant damage with unarmed strikes
- Servitor of the Black Hand Gloves - Deal additional 1d4Force damage with unarmed strikes
- Shadow-Cloaked Ring - Against obscured targets or creatures made of shadow, deal additional 1d4 damage with unarmed strikes
- Snow-Dusted Monastery Gloves - Deal additional 1d4Cold damage with unarmed strikes
- Thunderpalm Strikers - Deal additional 1d4Thunder damage with unarmed strikes
Bonus damage and interaction[edit | edit source]
- Flawed Helldusk Gloves - Deal additional 1d4Necrotic damage with unarmed strikes. On an unarmed hit, the target must succeed DC 13 Constitution save or suffer Bleeding for 2 turns
- Gloves of Soul Catching - Deal additional 1d10Force damage with unarmed strikes. On an unarmed hit, once per turn, regain 10hit points. Alternatively, you may choose to exchange the healing reaction for a +5 bonus to your next attack roll or saving throw, until the end of your next turn
- Helldusk Gloves - Deal additional 1d6Necrotic damage with unarmed strikes. On an unarmed hit, the target must succeed DC 15 Constitution save or suffer Bleeding for 2 turns
Bonus interaction[edit | edit source]
- Armour of Uninhibited Kushigo - While your Patient Defence is active, you can expend your Reaction to make an unarmed strike against an attacker that missed
- Hat of Uninhibited Kushigo - After dealing damage with an unarmed attack, gain a +1 bonus to spell save DC until the end of your turn
- The Sparkle Hands - Gain 2 Lightning Charges on each unarmed strike. Gain advantage on attack rolls against metallic creatures and armor while imbued with Lightning Charges
- Vest of Soul Rejuvenation - You can expend your Reaction to make an unarmed strike against an attacker that missed
