Overview[edit | edit source]

The Lower City Sewers is a large expansive area covering much of the Undercity and also serving as the main access point into various nearby areas. The sewers can be accessed through numerous manhole covers on the surface, and connects to almost every other location of the Undercity except the Murder Tribunal.
The boundaries of the Sewers are ambiguous because several locations share a similar aesthetic. For clarity, this wiki distinguishes between areas based on where the location name changes on the minimap. This applies to the Abandoned Cistern, the Guildhall, and the Undercity Ruins.
Historically a tribe of gutter kobolds was contracted to maintain free flow in the sanitation tunnels, but as of late they appear to have been slain or driven away by forces or circumstances unknown[1]. This is likely due to one or more of the following: members of the Cult of Bhaal, followers of the Stone Lord or the mad sorcerer Aelis Siryasius and his summons.
Entrances[edit | edit source]
There are several points from which the sewers can be entered. Most are outside somewhere in the Lower City and a few are from indoor locations.

Locations[edit | edit source]
The sewers are a vast network of tunnels and rooms which connect several places within Baldur's Gate. A subdivision is made of area's of interest inside the sewers.
Eastern Tunnels[edit | edit source]

The eastern tunnels of the sewers can be entered from Lower City in three ways, all are located on the far eastern part. One entry is on the northeastern side by entering a manhole located in Heapside Prison, one on the eastern side by climbing down a hatch in the Knights of the Shield Hideout, and one on the southeastern side by climbing down a manhole located in a side street between Stormshore Tabernacle and Basilisk Gate Barracks.
The entrance connected to the Knights of the Shield Hideout leads to a small room which contains a table with Patient Log: Duke Belynne Stelmane lying on top, describing Duke Stelmane's ongoing illness before her murder.
The entrance from the manhole in the side street initially leads to a room without an exit, but a wall can be broken down. From the room going west and following the stairway is an entrance to the Guildhall.
All three outside entrances end up at the same tunnel, which contains Noxious Fumes originating from vents on the ground. These can be disabled by hitting a lever on the southern wall (for instance with a Ranged Attack). In the middle of the room are shelves with a Rustic Chest. Behind its lock ( DC 15 Sleight of Hand Check) are Invasion Plans for Baldur's Gate and various wines.
The room can be exited on the western side, leading to a different part of the tunnels. Here cultists of Bhaal are fighting refugees. If the party have made a deal with Orin as part of Rescue Orin's Victim, they are not immediately hostile and the party can decide to join in, side with the refugees, or ignore them. On either side of the room are levers which disable the vents on the ground which are spreading Noxious Fumes.
West of the room are doors which lead to a hallway containing Tripwire killing potential fleeing refugees. At X: -60 Y: 784 is a hidden door which can only be opened from the other side, the area before the Secret Sewer Vault. The party can only get there by going through the Guildhall and into Nine-Fingers Keene's bedroom. Opposite to the hidden door is a short pier at which the sewage raft can dock.
Continuing the path further west will lead to another door which opens to the main sewer area. The 
Guildhall[edit | edit source]

Base of operations for Baldur's Gates' infamous criminal faction, The Guild, headed by Nine-Fingers Keene. There are two places where the Guildhall connects to the Lower City Sewers.
- One is a locked door called No Entry - No Exit at the back of the bar at X: -14 Y: 772 or - when coming from the sewers - the door is located at the end of a stairway coming from the southern most entrance as described at the eastern tunnels. It requires Severn's Sewer Key or can be lock picked with a DC 20 Sleight of Hand Check.
- The other one is a hidden door which can only be opened from inside the Guildhall near the Secret Sewer Vault.
Arabella's Cave[edit | edit source]

West of the 
If the conversing party member passes an Arcana check, they learn that the stone is magical, and has absorbed many lifetimes of stories from the city above. Arabella is now absorbing them too. She tells the party that "Bone Man" (her name for Withers) was right: the Weave guides her. She hints that the party might see her again. Arabella can be recruited as part of the Gather Your Allies quest.
A buried Wooden Chest is at X: -127 Y: 772, revealed by a DC 15 Survival Check.
North of the cave and near the initial bridge the ground is covered in Grease and Oil. A Traveller's Chest on the ground contains Parchment and various alchemical ingredients. Further north is another bridge which leads back to the main sewer area.
Sewage Raft[edit | edit source]

Inside the sewers is a raft which can be used to travel between three piers. Its initial position is at the middle pier which is locked behind a door ( DC 10 Sleight of Hand Check) at X: -78 Y: 827 just a little south of a ladder entrance located underneath a manhole in front of Golbraith's Cellar.
Next to the pier, the room behind the locked door may also contain Kith'rak Voss and Qudenos[2]. Whether or not they are here depends on the state of Help Kith'rak Voss. If the party attack them in the sewers, the party are called traitors and both will try to teleport away on their turn, even if restrained, silenced, or incapacitated. However, both can still be killed within that single turn.
The small raft on the pier has two levers, each indicating to which side it will traverse. Going south will bring the raft to the eastern tunnels which can also be reached by walking. More interestingly, the raft can be used to traverse up north to a pier where Lorna Esthelian stands. She is waiting for her date to take her into Cazador's Dungeon. The door to the dungeon called Gloomy Descent lies a bit further west of the pier and can only be accessed from inside the dungeon itself.
The pier is littered by several missing child posters, Creased Missing Poster, Curled Missing Poster, Damp Missing Poster and Faded Missing Poster. All reference children of the Gur abducted by Astarion in service of Cazador.
Sarin's Funeral[edit | edit source]

At the middle of the sewers the area starts to get covered in Grease, and several dead Absolute Cultists are around. Multiple Grease Mephits and Greaseballs patrol the area.
Crossing a stone bridge leads to the funeral area. When the party enter the first time, Aelis Siryasius warns them to halt and accuses them of working with the Cult of the Absolute. He is guarded by two Grease Elementals. The party can try and convince him to stand down or attack him. If the party fails any check, Aelis attacks. If they pass any of the checks, Aelis allows safe passage, though he cannot be spoken with again. If they later re-enter the sewers and approach Sarin's skeleton, Aelis becomes hostile.
On the other side of the bridge Sarin's skeleton lies in a coffin. It is missing its head. The skeleton's body has an interactable slot to reattach Sarin's Skull. Upon doing so Sarin's ghost bows and rewards the party with Chancer's Carcanet. Once the skull is returned to the skeleton, it stops glowing and is renamed Sarin at Peace.
Behind the coffin is a locked Grated Iron Door which can be unlocked with a Key carried by Aelis. Behind the gate is a Wooden Chest containing Scorched Book, gems, ingredients and gold.
Going north via the funeral, or through the eastern route parallel to the sewer brings the party to the same point: an area surrounded by closed doors which serves as entry way towards the Undercity Ruins. The party can also travel further west towards the treasure room.
Undercity Ruins Entryway[edit | edit source]

An entryway towards the Undercity Ruins is located in the northern part of the sewers. Several doors allow free entry to the area. Four loose stones are in the room. All require a successful perception check to spot and to be able to loot.
- DC 20 Perception Check at X: -131 Y: 880, contains Painted Key which is used to unlock a nearby Pained Chest
- DC 15 Perception Check at X: -133 Y: 894
- DC 10 Perception Check at X: -131 Y: 880, contains Bronze Ring and Thieves' Tools
- A check that will always succeed at X: -146 Y: 902, contains Incomplete Journal and gold
A Torn Journal Fragment lies on floor near the doors providing background information on Sarin and her companions. A locked Painted Chest is next to it, opened with its key or by lock picking. It can be looted for gold, a Studded Leather Armour, and a Rare and Uncommon arrow. In northwestern corner lies a Stained Journal Fragment together with a grease bottle. Nearby is a backpack with You've Got Friends in the Guild.
Going north leads to the Undercity Ruins and its related 
Undercity Ruins[edit | edit source]
While the party is inside the Lower City Sewers, the minimap label changes to "Undercity Ruins" by walking north through a stone archway at roughly X: -138 Y: 907. The 
The trader Voiceless Penitent Bareki is standing near the waypoint, behind him is a Pouch which contains Sarin's Skull which is needed to bring peace to Sarin, see Sarin's Funeral.
Treasure Room[edit | edit source]

The treasure room is a large open room which mainly features a large pipe going straight through the room and ending at an entry point for the sewers. Atop the slippery hill at the end of the pipe is a passageway called A greasy, smelly climb up... which links the sewers to the basement of Flymm Cargo.
On a piece of land surrounded by flowing sewage Dairow Vin lies unconscious. Grieving the loss and probable death of his husband, Dairow consumed a large quantity of Potions of Sleep, falling into an impenetrable slumber.
A Kobold Thief is on the other side of the pipe at X: -171 Y: 854. It carries a map of the sewers which references Bugthimble and Dragon. Tribe Treasure., a pointer to the final location of the treasure.
Upon reading the map, a marker shows up on the minimap called Cache indicating one of the five surrounding Dirt Mounds. Digging up the chest triggers a DC 25 Perception Check, which can reveal the chest is trapped. Each of the other dirt mounds has a similar trap and provoking note. The notes which can be dug up are

The actual treasure can be found in the most northern dirt mound at X: -178 Y: 862. From it the Marked Chest: Dragon is revealed which is also trapped and requires a DC 20 Sleight of Hand Check to lock pick. It contains gold, a Gold Necklace, and a Glass Goblet.
Following the large pipe in the opposite direction from the entrance leads to the Abandoned Cistern. The cistern can also be accessed by going through a broken gate inside the streaming sewage on the west.
Abandoned Cistern[edit | edit source]

The abandoned cistern is a hideout for the followers of the Stone Lord. It can be entered via the sewers either from the western door at the entryway to Undercity Ruins, or via the treasure room. The cistern lies in the western most part of the Lower City Sewers and has an entrance from outside the sewers in its western most point, up some stairs. The entrance ladder connects it to the Gardener's Basement, part of Lady Jannath's Estate.
The area is divided in two rooms, unlocking the western most requires solving a puzzle[4]. In it Minsc and Roah Moonglow are making deals with Bhaalists and doppelgangers.
Involvement[edit | edit source]
If Orin's kidnapping as part of Rescue Orin's Victim has not been triggered through a long rest, it can also be triggered in the Lower City Sewers. In this case, the event is mostly the same for each person - the kidnapped victim appears grievously injured and begs the party to run away, explaining how they were brutally tortured by Orin. Then, Orin reveals herself and the kidnapping plot.
This event is triggered in two places - in the sewers accessed from the Knights of the Shield Hideout or in the area close to the Undercity Ruins waypoint at X: -141 Y: 923.
