A variety of items, equipment, and characters from previous Baldur's Gate games make appearances in Baldur's Gate 3. However, in most cases the abilities of the items are slightly different because of the change of ruleset from 2e to 5e.

In Kagha's chest a book can be found written by Faldorn (human druid, companion in Baldur's Gate 1)
In the room where Minsc can be recruited in the Lower City Sewers there are several items where the following companions are mentioned:
- Dynaheir (human wizard, companion in Baldur's Gate 1),
- Mazzy Fentan (halfling fighter, companion in Baldur's Gate 2),
- Aerie (elf cleric/wizard, companion in Baldur's Gate 2),
- Jan Jansen (gnome wizard, companion in Baldur's Gate 2) and
- Keldorn Firecam (human paladin, companion in Baldur's Gate 2)

Locations

Although the layout of the city of Baldur's Gate is similar to that of previous games, some locations have changed slightly, or are in a different location.

Returning Locations

The Blushing Mermaid - Although a bar by the same name^[24] previously existed in the Upper City, this version is uncannily similar to the Low Lantern^[25], which was also a bar within a ship in the Lower City.
Elfsong Tavern^[26]
The Counting House^[27] - Formerly the Merchant's League Counting House, in the Easternmost part of the Lower City.
The High Hall - Likely corresponds to either the High House of Wonders^[28] or the Hall of Wonders^[29] from Baldur's Gate 1 (not the High Hall of the Radiant Heart in Athkatla)
Lower City Sewers
Iron Throne - the main antagonists' former headquarters, now submerged underwater
Ramazith's Tower^[30]
Sorcerous Sundries^[31]
Temple of Bhaal^[32]
Guildhall - Formerly the Thieves' Guild^[33]
Water Queen's House^[34] - also known as the Temple of Umberlee

— The Narrator if you try to go to the Shadowfell with your party scrabled

If Minthara does not trust that Dark Urge is in control of their actions there is an option to reply "An omnipresent authority figure controls all of my actions". This is a reference to Jaheira's line when you click on her portrait in the old games ("Yes, oh omnipresent authority figure?")

Act III

When Sceleritas reveals the Dark Urge's past to them at Wyrm's Crossing, he recites a twisted version of a Friedrich Nietzsche quote, which was used in the opening cinematic of the first Baldur's Gate game.

The Whispering statues in the Bhaal Temple recite Alaundo's prophecy from the previous games.
- Whispering Statue: The lord of Murder shall perish...
- Whispering Statue: ...but in his doom he shall spawn a score of mortal progeny.
- Whispering Statue: Chaos will be sewn from their passage.
- Whispering Statue: So sayeth the wise Alaundo.
Viconia's battle cry is a direct quote from her old games' battecry. (L'il Alurl! For Shar!)
If Lae'zel chooses to go against Vlaakith Gale will comment "Glad to see I'm not the only one on the wrong side of an opnipotent authority figure." Once again this is a reference to Jaheira's voice line in the previous games.
A grave X: 26 Y: 13 in the Lower City mentions Durlag's Tower which was the big dungeon from Tales From the Sword Coast, the expansion to Baldur's Gate 1.
A grave X: 31 Y: 13 in the Lower City mentions Branwen (misspelled as Branwem on the grave) who was a cleric companion in Baldur's Gate 1. She was also in the tutorial of Baldur's Gate 2, teaching the player about clerical abilities.
