Mind Flayer Tadpoles are Aberrations used by the Illithid to transform humanoids into Illithids through the process of Ceremorphosis. The player character and the origin companions of Baldur's Gate 3 are infected with tadpoles. One of the first main Quests is to .

It is revealed in Act One that the Cult of the Absolute is related to the Illithid abductions. By seeking out different healers to find a cure, the party can learn more about their affliction. It is also revealed that it is possible to absorb more tadpoles, unlocking Illithid Powers.
If Auntie Ethel or Halsin are spoken to, they reveal that the tadpoles are protected by shadow magic and cannot be removed by normal means. If initiating , Omeluum reveals their mind flayer parasites are held in stasis.
