- Astarion dreams about his past with Cazador Szarr and how the vampire lord forbade him from drinking the blood of sentient beings. When Astarion wakes up, he realizes he is free from Cazador, allowing him to drink the blood of one of the companions. Choosing to do so, prompts a skill check. If Lae'zel, Shadowheart or Wyll are chosen and the skill check succeeds, Astarion is able to feed and must pass a Saving Throw or the affected companion permanently dies. If the skill check fails, they wake up and confront Astarion about his vampirism. If the Persuasion or Deception check are failed, the affected companion leaves the party. Gale always wakes up since Astarion cannot feed on him because of the Orb and Astarion does not bother with Karlach because of the infernal engine in her chest. While Karlach technically can be chosen as the victim and has lines for it if he is caught, doing so requires very specific actions such as not Long Resting until the party return to the Wilderness from the Shadow-Cursed Lands after she gets her second upgrade from Dammon and avoiding any companion confronting him on his identity.

Type: Origin ( The Dark Urge)
Missable: Conditional
Requirements:
- Cross the Forest river.

No matter the outcome of the dialogue, she stays the night at the camp, but is killed by the Dark Urge at night. The Dark Urge then has to either hide the body through succeeding a DC 12 Stealth or Strength Check check or convince their companions they were innocent through succeeding a Persuasion check. The difficulty class for the persuasion check varies depending on whether or not the player character cleaned the blood off and how they chose to do so. It is also possible to blame Astarion if they know he is a vampire. Doing so causes him to lose approval. Alternatively, the player character admit to the murder. This event is also played automatically if they travel to the Rosymorn Monastery Trail or Shadow-Cursed Lands.
- If Alfira is already dead or knocked unconscious on the day this event is triggered, the campsite is visited by Quil Grootslang instead, and she is then killed instead of Alfira. If Alfira was knocked unconscious she later can be met at the place where she was knocked out and, after the camp celebration, can also be encountered in the Last Light Inn in Act Two.
